Important Notice: By booking an appointment with us, you acknowledge that you have read, understood, and agree to all policies outlined on this page. A printed copy of these policies is available upon request at the time of your appointment.

Appointment Policy

Alora Med Spa Cancellation & Appointment Policy

We have a 48-hour cancellation policy that will result in a $25 fee or loss of
service on existing packages. Thank you for your cooperation.

A credit card is potentially required to secure an appointment for single or group package services. To avoid a full-service charge and ensure appointment availability for other clients, please
reschedule, or cancel appointments within 48 hours of service. Walk-ins are
accepted; however, we recommend booking ahead to ensure availability.

Refund Policy

The provider of services works with each patient to discuss treatment options and their objectives, and to thoroughly review likely outcomes, benefits, and risks associated with each treatment. Once services are purchased, they will not be refunded. 

 

However, to ensure our patients always receive the greatest experience, unused service values (cash equivalent for the remaining amount of a treatment package) can be applied to any other service for which the patient is clinically eligible as determined by the provider of services. Cash Equivalents are not transferable. There are no refunds on credit or cash just the equivalent given. The client has 24 months from the original purchase date to redeem the credit after it has been issued in spa credit for services. 

All treatment sales (example: Botox, Juvederm, Radiesse,
Restylane, Perlane, and Sculptra) are final; refunds or credits cannot be
offered once treatment is completed. 

For Skin Care Products,
all sales are final. However, should the patient have a reaction to one of the
skin products, it can be returned for a full refund within 7 days of
purchase. 

Exchanges

When you purchase a package of treatments, you will receive a discounted price. The discounted price requires full payment for the series before the first treatment date. We do not offer refunds on packages that have been purchased. Should you choose to cancel after you have begun your treatment series, The Spa at Alora Med Spa will honor the outstanding amount in the form of a gift card exchangeable for goods and services. Credit will be given on remaining funds after the full (not discounted) price of the treatment is debited.The client has 24 months from the original purchase date to redeem the credit after it has been issued.



Gift Card Policy

Gift cards are non-refundable and cannot be redeemed for cash. Gift Cards can be purchased from the front desk, online, over the phone, and by email. Gift Cards can be redeemed against all services and products and are valid for two-year period from the date of purchase. Please note the gift cards are neither refundable nor transferable. Gift cards cannot be replaced if lost or stolen. Instant gift certificates may be emailed to the recipient or printed.

Patient Financial Responsibility

Your photo-identification will be photocopied and stored in your medical record for your protection and security.
This account is self-pay, and payment in full is due at the time of each service.

Any other arrangements that may involve a payment plan or payment deferral of any kind must be made in writing with the office manager or business manager of the Practice. Verbal agreements are not acceptable or binding.

Debit, Master Card, Visa, AMEX, Discover, HSA/FSA, and cash
accepted. Gratuity may not include on credit cards. Gratuity is always
appreciated. You may choose to directly tip your professional staff. or you may
use the gratuity envelopes located at the front desk reception. *Prices subject
to change without notice.

HIPAA Privacy Policy

We are committed to treating and using protected health information about you responsibly. This Notice of Health Information Practices describes the personal information we collect, and how and when we use or disclose that information. It also describes your rights as they relate to your protected health information.
Understand that under the Health Insurance Portability & Accountability Act of 1996 (HIPAA), I have certain rights to privacy regarding my protected health information. I understand, however, that this information can and will be used to:
● Conduct, plan, and direct my treatment and follow-up among the multiple health care providers who may be involved in that treatment directly and indirectly (Practice);
● Obtain payment from third-party payers; and
● Conduct normal health care operations, such as quality assessments and physician certifications.

HIPAA Privacy Notice
The provider of services is required by law to maintain the privacy of confidential information and provide individuals with notice of its legal duties and privacy practices with respect to such information; The provider of services is required to abide by the terms of this Notice; and The provider of services reserves the right to change the terms of its Notice and to make the new Notice provisions effective for all confidential information that it maintains. Revisions to this Notice will be posted in the patient waiting area.

Privacy Policy

Understanding Your Health Record/Information

This notice describes the practices of Alora Med Spa and its
staff (collectively, “Practice”), and that of any physician or provider with
staff privileges with respect to your protected health information created
while you are a patient at Practice. Practice, physicians with staff
privileges, and personnel authorized to have access to your medical chart are
subject to this notice. In addition, Practice and physicians with staff
privileges may share medical information with each other for the purpose of
treatment, payment, or health care operations described in this notice.

 

We create a record of the care and services you receive at
Practice. We understand that medical information about you and your health is
personal. We are committed to protecting medical information about you. This
notice applies to all the records of your care at Practice.

Social Media Policy

We may photograph or video treatments for marketing purposes. Your consent will be obtained before any images are shared on social media platforms.

Card on File Policy

A valid credit card is required to be kept on file for all appointments. This card will be charged for late cancellations or no-shows.

HSA/FSA Card Use Policy

We accept HSA and FSA cards for eligible medical services. Please consult with your plan administrator to determine which services qualify.

Spa Finder Gift Card Policy

We accept Spa Finder gift cards for services. Cards cannot be redeemed for cash and must be presented at time of service. Each client is allowed only the use of two spa finders cards per visit. If you have more please contact.
